Wellness: The proactive process of making  healthy lifestyle choices, which creates the greatest chance of sustaining and/or improving our physical, intellectual/vocational, emotional/social and spiritual well-being.

Goal: To provide us with a measuring tool for self-evaluation regarding our health and wellness.

Objective: To help us identify any dimensions of wellness and self-love that we would like to enhance by empowering ourselves.

Assumptions: That no gradient be established to compare anyone to anyone or to any standard.

That we acknowledge and honour ourselves at whatever level we are presently functioning, without judgement.

That if you choose to fill out this questionnaire please do so with integrity and as accurately and honestly as possible.

What is necessary for me to be physically, emotionally, intellectually and  spiritually healthy?
